You will need an unexpired, doctor-signed copy of your prescription and your pupillary distance (PD). Your PD may be reflected on your prescription. Pupillary distance is the measurement between your two pupils. It can be entered as a combined measurement or individually, by right and left eye. If your recent prescription does not include the pupillary distance you can ask your optometrist, or you can measure it on your own with one of these phone apps; PDCheck AR for iPhone or GlassesOn for Android. 

Note: Contact lens prescriptions are different than eyeglasses prescriptions and cannot be used to make a pair of glasses.
